Asthma affects an estimated 30 million Indians, while chronic obstructive pulmonary disease (COPD), primarily caused by biomass fuel exposure and tobacco use, claims hundreds of thousands of lives annually. Add to this the burden of seasonal allergies, tuberculosis, pneumonia and now the long-term respiratory effects of COVID-19, and it becomes clear that lung health deserves far more attention than it currently receives. The encouraging news is that most respiratory conditions are manageable with proper diagnosis, treatment and environmental control. Understanding your lungs and common disorders

Your lungs are remarkable organs with a surface area roughly equivalent to a tennis court, designed for efficient gas exchange. With every breath, oxygen enters the bloodstream and carbon dioxide is expelled. The airways — trachea, bronchi and bronchioles — are lined with protective mucus and tiny hair-like cilia that trap and remove particles. When this system is compromised, breathing becomes difficult and the body's oxygen supply is threatened.

Asthma is a chronic inflammatory condition where airways become hypersensitive to triggers like dust, pollen, cold air, smoke or exercise, causing episodes of wheezing, coughing, chest tightness and shortness of breath. COPD is a progressive disease primarily caused by long-term exposure to irritants, leading to irreversible airflow limitation. Allergic rhinitis, while not directly life-threatening, significantly impacts quality of life and can worsen asthma control. Pneumonia and tuberculosis are infectious causes of respiratory illness that remain significant public health concerns in India.

The impact of air pollution on lung health

Outdoor air pollution, particularly fine particulate matter (PM2.5), penetrates deep into the lungs and enters the bloodstream, causing inflammation, oxidative stress and damage to lung tissue. Long-term exposure is linked to asthma, COPD, lung cancer, heart disease and premature death. Indoor air pollution from biomass cooking fuels — still used in millions of rural Indian homes — disproportionately affects women and children, causing chronic respiratory disease from early childhood.

During winter months, many North Indian cities experience severe smog episodes where PM2.5 levels exceed safe limits by tenfold or more. These conditions trigger asthma attacks, increase hospital admissions and accelerate lung function decline in vulnerable populations. Climate change is extending pollen seasons and increasing ground-level ozone, further challenging respiratory health. Protecting lungs requires both individual action and community-level advocacy for cleaner air.

Managing asthma effectively

Asthma cannot be cured, but it can be excellently controlled. The foundation of asthma management is a personalised action plan developed with your pulmonologist. This plan identifies your triggers, specifies daily controller medications (usually inhaled corticosteroids), and outlines what to do during worsening symptoms. Controller medications reduce airway inflammation and must be taken regularly, even when you feel well. Reliever medications, such as short-acting bronchodilators, provide rapid relief during attacks.

Inhaler technique is critically important — studies show that up to 70% of patients use their inhalers incorrectly, reducing effectiveness. Identify and avoid triggers: use dust-mite-proof bedding, keep pets out of bedrooms, avoid tobacco smoke, and monitor air quality before outdoor exercise.

Protecting your lungs from pollution and allergens

Use N95 or equivalent masks when outdoor levels are hazardous — cloth masks and surgical masks do not filter fine particles effectively. Keep windows closed during peak pollution hours and use air purifiers with HEPA filters indoors if affordable. Avoid burning garbage, incense or mosquito coils indoors, as these release harmful particulates.

For allergy management, keep indoor humidity below 50% to inhibit dust mites and mould. Vacuum with HEPA-filtered devices. Wash bedding in hot water weekly. Shower and change clothes after outdoor exposure during pollen season. Use saline nasal rinses to clear allergens from nasal passages. If symptoms persist despite environmental control, consult an allergist for immunotherapy or targeted medication.

When to see a pulmonologist

Seek medical evaluation if you experience persistent cough lasting more than three weeks, recurrent chest infections, progressive shortness of breath, wheezing, coughing blood, chest pain with breathing, or unexplained weight loss with respiratory symptoms.
